At Archuleta Integrated Healthcare (AIH), a care team takes an active role with you in your health and well-being. A variety of professionals may be part of your care team, depending on your health goals.
This is a new approach to care at this location. A range of options is available to you, depending on your unique needs:
- Primary healthcare: We offer care across all life stages to anyone who needs primary care. Our care includes wellness visits, family healthcare, immunizations and chronic illness management.
- Behavioral healthcare: We know that behavioral healthcare is an important part of your overall health. At AIH, we regularly check in with you to respond to any behavioral health needs you may have. We also offer a range of behavioral healthcare options, including counseling, substance use treatment, psychiatric evaluation and medication management. You can receive behavioral healthcare at AIH without changing your current primary care provider.

- Oral healthcare referral:We offer referral options to the Axis Health System Oral Health Clinic in Durango.
We are now accepting new patients, and offer a variety of payment options. Medicare, Medicaid, CHP+ and most insurances are accepted. A sliding-fee scale is available for those who qualify.
